I take NSAIDS (tylenol or ibuprofen) and hydroxychloroquine. It's moderately effective if I keep my sleep on schedule and stress as low as realistically possible.

It's completely normal for medications to become less effective over time with RA. Many myRAteam members have shared similar experiences of needing to change medications after they stop working effectively. If you're feeling that your current medications aren't helping, it's important to have an open conversation with your rheumatologist about your symptoms and concerns. Your rheumatologist may consider adding or switching to different types of medications, such as biologic DMARDs, which work differently than traditional DMARDs like leflunomide. Some patients may need combination therapy to achieve better control of their symptoms. The key is working with your doctor to find the right medication or combination that works best for your specific situation.
