So sorry, you're having so much trouble. I would ask The office manager of the doctor's office to do it got to help me with mine
That's all I get from rheumatologist is a temp pcp gives me perm but take that paper to your dmv and they will hand you your card right then...good luck
Thanks for the advice! I went thouugh the secetary and I just got the signed form today in the mail. However, it is a 6 month temporary letter. I called and the hospital said that they are not allowed to do the perminant ones. I am a little confused since my spondyloarthritis is only going to get worse with time. Do I get a license plate as well with the temporary pass? I thought it was going to be a permanent one. I have to call and they will resend the letter every 6 months. I guess my expectation was that it was going to be a "from now on" thing... Not a problem if it is not... just wanted to ask to be sure

The form for a handicap placard can be filled by your primary or your rheumatologist.
