My left knee didn't hurt until 2 yrs later. I thought it was because I was over compensating for my right knee. Then it moved to my right hip. At this point, I'm in excruciating pain. Nothing helps. I don't want to take any dmards/ or whatever they're called. Side effects are too severe to even want to chance them. Just shove more poison in my body. Not fun. It's just the pain is getting so much worse, I'm crying while texting from the pain. No pain in my hands, fingers or wrists. I saw a… read more

First — the pain you're describing sounds absolutely exhausting and overwhelming, and it makes complete sense that you're frustrated and questioning everything right now. That frustration is valid.

Here's the thing about RA — it doesn't always follow the "textbook" rules. While it typically starts in smaller joints symmetrically, many people experience it very differently. A myRAteam member once shared, "My pain started in my right knee before exploding to every other joint." Another said their flares "go from joint to joint, from one day to the next."

RA can also affect larger joints like hips and knees, sometimes without obvious swelling — internal inflammation doesn't always show on the outside. Regarding DMARDs — the concern about side effects is completely understandable. However, it's worth knowing that untreated RA inflammation can progressively damage joints permanently, and that damage cannot be reversed. The pain you're experiencing now may worsen without treatment.

There are different types of DMARDs with varying side effect profiles. It might be worth having an honest conversation with your rheumatologist specifically about:

- Which options have the mildest side effect profiles
- Starting at lower doses
- Non-biologic options first
- What pain management options exist in the meantime

You deserve real relief. Please don't suffer alone — reaching out to your doctor about how unbearable the pain has become is so important right now.
