That sounds really rough — feeling almost like yourself again and then crashing two hours later is genuinely frustrating. That sounds really rough — feeling almost like yourself again and then crashing two hours later is genuinely frustrating. What you're describing could very well be a known phenomenon called "methotrexate fog," which some people experience after a dose. It can bring on fatigue, headaches, and just feeling generally awful.

The good news is that this doesn't necessarily mean methotrexate isn't for you. Many people find these after-dose feelings improve over time as their body adjusts. There are also some practical strategies worth discussing with your rheumatologist that may help reduce how rough you feel after a dose:

- Timing your injection on a Friday or Saturday so you have the weekend to recover
- Taking folic acid (folate) supplements, which can ease side effects
- Using anti-nausea medications if nausea is part of what's hitting you

One myRAteam member shared that in the beginning they felt worse the day after methotrexate, but after about six months that problem went away entirely.

It's really worth talking to your rheumatologist before making any decisions about stopping or changing your dose — they can help figure out whether adjustments might make this more manageable for you.
