I just started methotrexate (3rd dose). Before it, I complained to my rheumatologist that my fatigue is unreal, besides having an increase in pain. She put me on methotrexate and said it can cause fatigue, but maybe you won't notice it, since you're already fatigued. Luckily, I haven't noticed a difference in the fatigue.

What happens the day after taking methotrexate?

Some people experience what's called "methotrexate fog" the day after taking their dose. This can include:

- Fatigue and feeling unwell
- Headaches
- Mental fog and difficulty concentrating

Gastrointestinal issues like nausea, vomiting, and diarrhea are also common side effects that may occur, affecting between 20-65% of people taking methotrexate for RA.

However, experiences vary. One myRAteam member shared that in the beginning, they felt worse the day after taking methotrexate, but after about six months, this problem went away.

If digestive problems or other side effects are bothering you, talk to your doctor. They might suggest taking methotrexate after a meal, scheduling your dose on Friday or Saturday so you have the weekend to recover, or taking a folic acid supplement to help reduce side effects.

Keep in mind that methotrexate takes time to work fully - usually several weeks to months - so initial side effects don't mean the medication isn't right for you.
