RA can affect more than just the joints β€” the lungs are one of the most commonly impacted areas outside of the joints. About 1 in 10 people with RA will develop some form of lung problem, and in some cases, lung symptoms can actually appear before joint pain does.

The inflammation that drives RA can also attack lung tissue, leading to scarring, fluid buildup, and other complications. Here are the main ways RA can affect the lungs:

- Interstitial Lung Disease (ILD) β€” The walls of the tiny air sacs in the lungs become scarred and thickened, making it hard to breathe. Symptoms include shortness of breath, a dry persistent cough, fatigue, and loss of appetite.
- Airway Blockage (Bronchiectasis/Bronchiolitis) β€” Inflammation can block the small airways leading to the air sacs, worsening breathing difficulties.
- Pleural Effusion β€” Fluid builds up around the lungs, preventing them from fully expanding and causing pain when breathing.
- Pulmonary Nodules β€” Lumps can form in the lung tissue. While not cancerous, a ruptured nodule can sometimes cause a collapsed lung.
- Pulmonary Hypertension β€” High blood pressure in the arteries between the heart and lungs, which can eventually strain the heart if left untreated. One myRAteam member shared: *"I can't do simple stretches and it's hard to breathe and sleep."*

It's worth knowing that some RA medications, like methotrexate, can also contribute to lung complications in rare cases. If you notice any new breathing difficulties, it's important to flag them with your doctor right away β€” they may want to adjust your treatment plan.

Staying on top of your RA treatment, quitting smoking if applicable, and keeping up with recommended vaccines (like the flu shot and pneumonia vaccine) are key steps in protecting your lungs.
