Does Methotrexate Wear Off In Between Doses?

A myRAteam Member asked a question 💭
Boston, MA

Hi everyone,
I take methotrexate (orally) on Friday nights. From Saturday to Wednesday, the pain is a lot less. I can also go down to 15mg of prednisone. By Wednesday night, I can already feel a difference and I need to go back up to 20mg of prednisone on Thursday morning or else I wouldn’t be able to put any weight on my foot. Is it possible that the methotrexate is wearing off?
Thank you!!

Members strongly validated the experience of methotrexate seeming to wear off before the next weekly dose, with many reporting increased pain and stiffness by Wednesday or Thursday that eases again after taking their medication. Several members shared practical next steps, including discussing a dosage increase with a rheumatologist, considering the addition of a biologic like Humira or Enbrel if methotrexate alone isn't providing sufficient control, and exploring complementary options like CBD oil to manage pain between doses. A recurring theme was the frustration of having only a few good days each week due to medication timing and side effects, balanced by gratitude that the medication provides at least some relief and hope that adjustments can extend that window of comfort.

Biologics are medications used to treat RA. I'm sure you've see the ads on TV for Humira and Enbrel. They are sometimes necessary to take in addition to methotrexate to treat the RA

Yes! I take it Saturday morning and by Wednesday I start to feel fatigue and pain again. Once I take it on Saturday it feels like it takes a day to get in my system again. Will talk to my doc about increasing the dosage...

Fell well soon!

The trexate is working... Yes, sometimes it slows down. It takes two weeks to flow out of your system. I have to take mine weekly as well and I find that by Wed, I'm ready for my Friday dosage.
How do you use prednisone for so long? It messes with my muscle functions...especially my bladder and kidneys? My doctors took me off after two weeks!!

Hi all. I used to be a blood donor but am unable to donate once taking methotraxate. I was advised I could start donating again three months after I stopped taking methotraxate. So it remains in your system for awhile
